Welcome to our latest newsletter. This month, as practices continue to take collective action, we have an update and advice on unfunded work. Our interface update has some positive indications of the impact collective action is having. There's also news on GPs and the ARRS scheme, and an offer of support for practices affected by the riots earlier this year.

Collective Action UpdateThank you to every practice now taking part in collective action to protect patients and practices. Practices can continue to take action using the safe sustainable action menu. It is up to each practice to choose which actions to take, and whether to do one, or many. None of them represent a breach of your contract, but they will help you manage your workload and keep you and your patients safe. The BMA have released a new short video on unfunded work. GPs are asked to consider serving notice on any voluntary services currently undertaken that plug local commissioning gaps and stop supporting the system at the expense of your patients, practice and staff.

Focus On... The Role of the LMC part 2

Dr Zoe Norris, Chief Executive Medical Director, continues a series of posts on the LMC.
Our work is probably evenly split at the moment between direct practice support – queries from GPs or practice managers, and system support. While Dr Rolan Schreiber, our Medical Secretary, tends to lead on clinical queries or pathway issues, I spend more time doing the contractual queries, partnership difficulties, or support for colleagues with complaints or performance issues.
We attend meetings with all our PCN Clinical Directors across the Humberside region, and link into the primary care collaborative and ICS primary care executive. Both of us then attend meetings to represent our practices at system level, that is within our 4 “Places” (North Lincs, NE Lincs, Hull and ERY) and with the wider Humber and North Yorkshire ICS.
Some of these meetings might be around patient removals, list closures or allocations, or quality concerns. Others are broader around outpatient transformation programmes and the strategic direction of the ICB. At all of these, our role is to make sure general practice isn’t forgotten. It’s to remind people that there are a range of practices and PCNs, of different sizes and make-ups who are all working incredibly hard trying to look after their patients.
We spend a lot of time explaining how the GP contract works, and reminding system partners that our practices are small businesses with personal liability and no chance to overspend their budget! There is a lot of relationship building that goes in, and we try to be seen as a firm but fair critical friend to the system. This means that our concerns are taken seriously, and the door is open at the early stage of any change for us to articulate the impact on general practice of an idea from another part of the system.

Support Offer from Humberside LMCs - Response to August 2024 Riots Affecting General PracticeThe LMC are in receipt of some ICB funding specifically earmarked to support practices that were affected by the far-right rioting and domestic terrorism that mainly occurred in Hull, but also spilled out into Grimbsy and Scunthorpe, back in August of this year. These events had both a direct impact on colleagues from minority backgrounds, and created wider distress amongst staff who live and work in those areas. It was recognised that there was a lack of immediate and tailored support and education that could be offered, and that the impact on staff extends beyond the short term. Our funding means we are able to offer two types of support – the first is aimed directly at minority colleagues, and the second is aimed at wider team support & training. If you feel that your team has been affected in this way and would benefit from some support, please get in touch with us directly and we can discuss your requirements and what is available in more detail. GP Additional Roles Reimbursement Scheme (ARRS)The updated PCN DES bringing in the GP ARRS was released this month. GPs employed via the Scheme must be within 2 years of their CCT on 1/10/24 and PCNs will be required to provide terms no less favourable than the BMA salaried GP model contract, in line with the GMS/PMS contract. GPC England and the Sessional GPs Committee have released guidance for PCNs, and individuals employed under this scheme. It can be found here.

Free eLearning Module - Tourette SyndromeThe charity Tourettes Action has launched a new learning module, developed with clinicians, to learn what Tourette syndrome is, the symptoms of the condition, co-occurring features and differences that accompany it, differential diagnosis and support and specialist input. This course is aimed primarily towards GPs. The course can be found on their website here or the NHS learning hub here.

Community Urgent Eyecare Service (CUES) The Community Urgent Eyecare Service (CUES) provides urgent assessment, treatment or referral for sudden onset eye problems such as flashes, floaters, vision loss or minor eye injuries. Patients should be signposted to the CUES website https://primaryeyecare.co.uk/services/urgent-eyecare-service/ to find a local optometrist practice.

Huntingdon's Disease Webinar for GPsFriday 24 January 2025, 13:00-14:00 OnlineA free webinar by the Huntingdon's Disease Association. The role of the GP is crucial in ensuring people living in families where there is Huntington's disease receive the right support at the right time. Providing the right care for people with Huntington's disease can be challenging, as needs change throughout the course of disease progression. Support also needs to be given to family members who are at risk of inheriting the gene and may be considering being tested to see if they too will go on to develop symptoms. At the moment there is no cure. BMA GP Opinion Survey 2024This annual survey gathers GPs’ views on current issues and opportunities facing general practice, helping to influence and inform this year’s contract negotiations in addition to the longer-term strategy of promoting and protecting the future of general practice. The survey is open to all GPs in England, including partners, salarieds, locums, and GP registrars at ST3 and above. It is open to GPs working in all settings and practices, wider primary care roles, trusts, urgent care, and secure or out-of-hours settings. Participants do not need to be a member of the BMA but will need a GMC number to ensure that responses are coming from eligible GPs based in England. All responses will be anonymised. New Dispensing FeescalesThe new dispensing feescales for England and Wales, effective from 1 October 2024, have been published, showing a reduction in the average dispensing fee to 218.7p per item (average), a decrease of 0.2p compared with April 2024. This reduction is based on a new methodology designed to further smooth out the fluctuations seen in the previous biannual fee adjustments. Dispensing fees are intended to cover the costs of running a dispensary, including staff expenses. However, from April 2025, the average fee is set to reduce further by 5.37p per item (on average) to 213.3p (compared to the October 2024 rate). This continued downward trend may prove to be a tipping point for some dispensing practices, as the cumulative impact of underfunding in rural practices takes its toll. GPC England continues to work closely with the Dispensing Doctors Association; both organisations recognise and promote the value of dispensing practices to their patients and within their communities.

Humberside LMCs and the NHS People Promise - Practice Nurse Forum We held our first virtual forum for practice nurses from the Humber area at the beginning of the month. We were delighted with the attendance and engagement and really look forward to working with practice nurses over the coming months. We will be joined by Dr Aarti Bansal, GP and Clinical Net Zero Lead for Humber and North Yorkshire ICB for our next forum on Tuesday, 10 December, 12:30 to 13:30. Join the forum via MS Teams. We Need to Talk ConsultationThe local NHS across Humber and North Yorkshire has launched We Need to Talk – a conversation with people through social media and in-person focus groups about the future of the NHS in our area. The process will lay bare some of the difficulties faced by the NHS, whilst giving people an opportunity to signal how they might want the NHS to change to meet the challenges of today and the demands of the future. In a snap poll carried out by NHS Humber and North Yorkshire Integrated Care Board (ICB) ahead of the public engagement, almost 63 per cent of the 428 people who responded said the NHS needed to change. GP Contract Changes and Implementation The following elements have been uplifted from this month: Global sum has been uplifted to £112.50 per weighted patient for GP practices to implement the DDRB uplift. Locum reimbursement rates have increased, along with the pay elements of workforce-related transformational funding. From 1 October, newly qualified GPs can be recruited as part of the additional roles reimbursement scheme (ARRS) and PCNs can request the funding as outlined in the updated PCN contract DES (see above). In addition, three other PCN funding streams have been increased to reflect the DDRB uplift: core PCN support, the care home premium service and the enhanced access service. The maximum reimbursement rates for existing ARRS staff have been uplifted with effect from 1 October to align with Agenda for Change pay scale uplifts.

GP Registration GMS1 Form Phased Out‘Register for a GP surgery’ is a free service for GP practices across England which simplifies online patient registrations. All GP surgeries in England must offer the national service from 31st October 2024. As part of this change, the GMS1 form will be phased out and replaced with a new paper registration form (PRF1) for those patients who are not able to register using the new digital service. The PRF1 reflects the new online registration form that has been developed. The GMS1 form will not be available to order from the PCSE website from 31st October 2024. Vaccination Data Dashboard – Winter 2024 Campaign UpdateThe Vaccination Data Dashboard has been updated to support the main launch of the autumn/winter Flu and COVID-19 campaigns, alongside co-admin capabilities. This provides GP surgeries with a view of their eligible patients, national invite status and their seasonal vaccination status to support local call and recall. Frontline Healthcare Worker Winter Vaccinations Eligible frontline health and care professionals may be offered a COVID-19 vaccination by their employer or can book a vaccination appointment online. The seasonal flu vaccine is available to all eligible frontline healthcare workers, including clinical and non-clinical staff, through their employer. Find out more in the seasonal vaccination letter. Communications materials to support winter vaccination uptake for frontline healthcare professionals can be downloaded from the Campaign Resource Centre. GP Pressures: Latest Workforce and Appointments DataGPs continue to treat many more patients than they have done in the past. The latest data shows a record 63.47 million patients registered with practices in England, while there are over 1,000 fewer GPs compared to 2015. As a result, each full-time equivalent GP is now responsible for an average of 2,282 patients, 345 (or 18%) more than in 2015, and appointments level at an average of almost 1.5 million per day.
